  • AS OF MARCH 10, 2014 DR. GILL WILL ONLY BE IN THE OFFICE MONDAYS and SATURDAYS.
  • IN ORDER TO DECREASE A LONGER WAIT, IT IS VERY IMPORTANT TO KEEP YOUR APPOINTMENT TIME AND BE SURE TO ARRIVE ON TIME.
  • DUE TO THE HIGH DEMAND FOR MEDICATION SERVICES, APPOINTMENT TIMES ARE LIMITED AND ARE SUBJECT TO CHANGE.
  • NO MEDICATION CHANGES WILL BE MADE OVER THE PHONE.  NO MEDICATION WILL BE PRESCRIBED OVER THE PHONE.
  • ALL PATIENTS MUST BE ACCOMPANIED BY THEIR LEGAL GUARDIAN TO EVERY DOCTOR APPOINTMENT.
  • IT IS YOUR RESPONSIBILITY TO SCHEDULE A FOLLOW UP APPOINTMENT AND ADDRESS ANY CONCERNS REGARDING THE MEDICATION BEFORE LEAVING YOUR APPOINTMENT.  DOING THIS WILL HELP AVOID THE CHANCES OF RUNNING OUT OF MEDICATION BEFORE YOUR NEXT FOLLOW UP.
  • YOU MUST SEE YOUR THERAPIST WEEKLY OR EVERY OTHER WEEK IN ORDER TO RECEIVE/CONTINUE RECEIVING SERVICES FROM THE DOCTOR.
  • IF YOU HAVE MORE THAN TWO NO SHOWS, SAME DAY CANCELLATIONS OR IF YOU HAVE NOT BEEN SEEN BY THE DOCTOR FOR MORE THAN SIXTY DAYS YOU WILL BE AT RISK FOR DISCHARGE FROM MEDICATION SERVICES.  PLEASE GIVE AT LEAST 48 HOURS NOTICE IF YOU WISH TO CHANGE A SCHEDULED APPOINTMENT.
