If you are one of our future clients that has been referred for services through Fresno County and would like to schedule an appointment, please contact our office at (559) 256-2000 between the hours of:
8:00am and 7:00pm Monday – Thursday
Fridays 8:00am -5:00pm

In order to speed up the scheduling process please have access to your/your child's Medi-cal number, date of birth and/or social security number.

CPI understands your time is important, which is why we have some of our paperwork available to you online. 

We ask that you review and complete documents prior to your appointment.

Please inform the office if you would prefer these documents mailed directly to you.

Please be sure to bring all intake documents with you at the time of the assessment. If there are questions or concerns that have made it difficult to complete the intake packet, please plan to arrive 30 minutes prior to the appointment so that our staff may assist you.

At the time of your appointment you will need to bring in the following:

  • The client’s Medi-Cal card and any other insurance information.
  • Any and all court documents identifying you as the legal guardian.

We are happy to report that most assessments are scheduled within 10 days. If you have questions, please call us at (559) 256-2000.
